Instructions for submitting for FAWM
credits
1. Educational courses and
conferences previously attended qualify for FAWM Credit.
Registrants must have either already been a WMS member or
had become a member before leaving the event. Conferences
and other activities that qualify for either current or
retroactive review for credit are listed on the eligible
events page. Activities conducted with organizations
that are or were NOT affiliated with the WMS are not eligible
for FAWM credit. The FAWM application information explains
what must be done to be awarded retroactive credit. Registrants
must request review for retroactive credit before December
31, 2005 and you must have been an active WMS member at
the time the courses were taken. Activities must not have
occurred before January 1st, 1999.
2. FAWM Registrants may determine
which activities are eligible for either retroactive or
current FAWM credit by going on line to the Eligible
Course page. This is a comprehensive listing that will
be updated as organizations affiliate with the WMS.
3. Registrants may use the
Academy web site to efficiently plan educational experiences
and add to their FAWM credits at the same time. They can
also determine in advance for which and for how many FAWM
credits a particular activity has been evaluated. Click
on the “Enter and Edit Credits” link above.
A separate window will appear that will require a “log
on” with last name and WMS membership number. A drop
down menu will appear with all of the eligible activities.
Select one of interest and it will present a list of sessions
to be conducted at that activity and their respective FAWM
credit evaluations.
4. Registrants must submit the appropriate
information to receive FAWM credit for sessions at activities
they attended.
Step 1: Submit a hard
copy (fax or snail mail) of a completion or CME certificate
or similar official document of each educational activity
for which you are requesting FAWM credit. These documents
are kept on file at the administrative office with your
application materials. Upon receipt of these documents,
your FAWM registry account will be activated for those
particular activities and made accessible on-line.
Note: This step is required even though
you may already be on electronic record at the WMS as
having attended an activity.
Step 2: Click on the
“Enter and Edit Credits” link above. A separate
window will appear and will require a “log on”
with last name and WMS membership number. A drop down
menu will appear with all of the eligible activities.
Select the one you attended and it will present a list
of sessions that were conducted at that activity and their
FAWM credit evaluations. Click “Yes” next
to each session that you attended. Click on “submit”
at the conclusion of each activity. A summary screen will
appear. The data base will automatically tabulate your
FAWM credit status according to the Academy guidelines
and the number and type of credits offered for each session
that you entered.
Note: You will not be able to enter
or edit credit information for activities for which you
did NOT submit a hard copy document or that was NOT affiliated
with the WMS.
5. Registrants may check the status of their
accumulated FAWM credits on line.
Click on the “View Credit Status” link above.
If not already logged in as a member, a log in page will
appear. After logging in, a general summary of your FAWM
credit status will appear. This summary is organized by
FAWM curriculum topic and NOT by sessions that were submitted.
A more detailed review organized by Academy Topics and sessions
submitted listing what was awarded for each may be accessed
by clicking the “View Submission Details” link
above.
